Community Support Specialist Overview:
The Community Support Specialist at ReDiscover will provide assistance to people with severe and persistent mental illness to live successfully in the community.
The Community Support Specialist can expect to be a vital asset to ReDiscover clients by utilizing:
Strategy & Planning Facilitating each clients achievement of long range outcomes of symptom management, employment and independent living based upon the needs of the clients.
Engages difficult to serve clients by investigating all potential resources in order to gain access and make connections.
Assesses clients completely using all the assessment tools required to develop a comprehensive assessment.
Ensures that treatment plan objectives, interventions, services provided, and documentation are related to needs identified within the assessments and are medically necessary.
Routinely identifies warning signs and symptoms, responds to the changes, and educates clients on symptoms management.
Identifying and providing clinical interventions and referrals for clients based on their needs.
Empowering clients to develop and nurture the knowledge, skills, and support systems crucial to promoting optimal functioning in the community Continually assess clients for lack of progress, progress, or emergent needs and address those needs clinically.
Operational Management Working with all internal members of the primary treatment team as well as the department treatment team to ensure positive communication regarding the coordination of client care.
Working with external providers to ensure positive communication regarding the coordination of client care.
Support the strategic goals and mission/vision/values of ReDiscover when interacting with clients, coworkers, and community stakeholders.
Safeguards Protected Health Information written and electronically and maintains confidentiality.
Contribute to the mission of ReDiscover by adjusting work priorities and duties as needed to meet unforeseen demands.

Bachelors Degree from an accredited college or university in a social science related discipline.
Qualifying experience may be substituted for education.
A working knowledge of mental illness.
One year of direct service to the target populations or other related human service experience preferred.
Good written, oral, and interpersonal communication skills.
Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as GSuite.
High integrity by observing ReDiscover policy/procedures, accreditation standards, laws, and ethics.
Self-driven individual with a customer service focus.
Team-oriented and skilled in collaborating with others.
A valid automobile operators license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work hours.
