We are seeking a full-time long-term care social worker that will provide assessment, advocacy, and discharge planning for the psycho-social needs of the patients of all ages.
This individual will need to demonstrate expertise in crisis intervention, mandated reporting, care coordination of patients with psychosocial risk.
Must be able to prioritize multiple tasks and possess strong interpersonal skills.
Must have excellent communication skills and display a professional demeanor with patients, families, interdisciplinary team members, and outside agencies.
A Social Services Director is responsible for coordinating the psychosocial needs of residents and their families.
This team works closely with the Nursing Department to meet needs of residents.
Social Services Director Requirements:
Bachelor's Degree in Social services Certification as a CSW (Certified Services Worker) by the state of Missouri Knowledge of community resources Experience in Health Care Social work 2-5 years of acute or long-term care experience preferred.
MDS experience a plus.
Experience in long-term care social work also a plus.

Estimated Salary: $20 to $28 per hour based on qualifications.
